Your Aged
Care Guide

What you need to know

Navigating the aged care system can seem daunting. It’s a complex web of services, subsidies, and assessments, but rest assured, you’re not alone. At My Care Journey, we’re here to guide you through each step with clarity, empathy, and personalisation.

The Australian Care System

The Australian aged care system is designed to support older people and are divided into two primary types of care; home care (in-home) and residential aged care (aged care home).

Home Care is for those wishing to stay in their own homes, offering a range of services from personal care and domestic assistance to nursing, allied health, and social support.

Residential Care caters to those requiring 24-hour support, providing accommodation for older people who are unable to live at home safely or independently.

Both types of care receive subsidies from the Australian Government under the Aged Care Act 1997. Nevertheless, individuals are expected to contribute to their care costs when financially feasible, with the amount of subsidy and personal contribution varying based on the care level needed, as well as the individual’s income and assets.

Whether you are looking for Aged Care Homes or In-Home Care, we’ll help you find the provider that is the right fit for you!

Assessing Your Care Needs

To qualify for subsidised aged care, older people must undergo an assessment by an Aged Care Assessment Team (ACAT), or an Aged Care Assessment Service (ACAS) in Victoria. This evaluation determines the person’s eligibility and level of care needs, resulting in an approval letter detailing the types of care they can access and their referral code.

Choosing the Right Care Provider

Post-approval, individuals can select a provider that aligns with their preferences, budget, and location. You can directly contact providers to explore your care service choices. My Care Journey streamlines the process of finding suitable providers by presenting your top care choices based on your needs, preferences and budget, free of charge!

Starting Your Care Journey

Before receiving care, it’s essential to sign a care agreement with the chosen provider, detailing the services, fees, and both parties’ rights and responsibilities. An income and assets assessment form is also necessary to calculate the individual’s contribution to care costs. Online calculators or fee estimates from providers can assist in understanding these costs before signing the agreement.

Remember, you’re not locked into one provider or type of care forever. It’s possible to change providers or care types if you’re unsatisfied with the current arrangement or if your needs change over time. Temporary respite care in a residential facility is also an option for those needing a break from their usual care routine.

At My Care Journey, we’re committed to making your care journey as smooth as possible. We’re here to guide you through these steps and help you make informed decisions in the spirit of transparency. Whether you’re starting your care journey or need assistance navigating the system, remember, you’re not alone. We are here to help you.
